We’re inviting applications for a Population Health Fellow to join us for 7 hours per week (2 session) over 12 months, starting no later than 1 March 2026. This opportunity is open to senior clinicians or administrative/management professionals working in Somerset general practice. The role is remunerated at the equivalent of Agenda for Change band 8a (mid point).
About the Fellowship
The NHS Health Check aims to provide early detection of cardiovascular disease for people 40-74 with a once every five year check for people who do not already have a cardiovascular condition. LD and SMI annual health checks provide health check opportunities to groups with recognised inequalities in health outcomes. A goal of this fellowship is to progress alignment of these two annual health check programmes to widen inclusion of the NHS Health Check programme and make best use of resources. An existing ICB project is also looking at increasing uptake of annual health checks and this fellowship should collaborate with this workstream.
The Fellow Will:
Bring expertise and understanding of implementation of NHS Health Checks and SMI and LD Annual Health Check programmes. Link with other projects on SMI and LD Annual Checks
Progress the understanding we have at a local level on the ability to address secondary cardiovascular outcomes and other public health outcomes within those with a learning disability and or severe mental illness.
Work towards an implementable business model to increase the reach of NHS Health Check to those on either the SMI and or LD register or other groups who are less well reached by the current delivery model.
The Ideal Candidate Will:
- Be a senior clinician or experienced administrator/manager working in Somerset general practice.
- Have experience or interest in working with SMI and or LD populations
- Be skilled in project management and IT.
- Be an excellent communicator and collaborator, able to work across teams and settings.
- Be passionate about improving population health and patient empowerment.

Support: Mentorship from the Somerset Training Hub and relevant clinical/public health leads.
