What will I be doing?
The School Transitions Officer is responsible for actively working with local schools and external agencies to support young people in making a successful transition into college, playing a vital role in student recruitment, progression and retention. The postholder will build and maintain strong relationships with local schools, parents/carers, young people and key stakeholders, including wide range of specialist schools, to coordinate structured and supportive transition pathways for young people with SEND needs and EHCP’s. There will be a specific focus on primary Schools in the Sedgemoor area, but this will need to be flexible on need and the role requires work across all four USC Colleges. The School Transitions Officer will be a key point of contact for schools and will attend planning meetings and EHCP annual reviews to gain a clear understanding of potential students. To be able to support offer guidance to learner, particularly those with additional needs or vulnerabilities, towards suitable courses and to provide college curriculum with key information on learners. This will include arranging taster sessions, and facilitating robust, learner-centred transition plans. In addition, the postholder will play an integral role in the EHCP consultation process and proactively identify and respond to individual student needs, providing effective, solution-focused support to enable positive outcomes.
